Refined Youth Group Liability Waiver, 2026

I, (parent/guardian), am the parent or legal guardian of [youths(s)] (Participant), and I voluntarily consent to their participation in all Refined Youth Group activities during the 2026 program year.

I understand that participation in activities organized by Refined Youth Group may involve physical activity, travel, and other situations that carry inherent risk of injury, illness, or property damage. I voluntarily assume all such risks on behalf of my child.

On behalf of myself and my child, I hereby release and agree not to hold liable Refined Youth Group, its officers, employees, volunteers, and agents (Released Parties) for any claims, damages, or losses arising from my child's participation in any 2026 activity. I further agree to indemnify and hold harmless the Released Parties from any costs or claims arising from my child's participation.

I authorize Refined Youth Group staff to seek and consent to emergency medical treatment for my child if I cannot be reached in a timely manner. I accept financial responsibility for any resulting medical costs.

I consent to Refined Youth Group using photos or recordings of my child for organizational purposes (see opt-out below).

I have read and understand this document and am signing it freely and voluntarily.
